Dear MCH:

As a requirement of the MCH Program, you are required to re-certify on a yearly basis. The following is a refresher of the 1 year re-certification process.

Each MCH will be required to re-certify every year. This will be accomplished by accumulating five (5) re-certification points.

The ways in which you may receive these credits are listed on the enclosed re-certification form.

The annual re-certification fee is MNLA Member $20.00.

The annual re-certification fee is NON- MNLA Member $25.00.

To avoid any interruption in your status of MCH, your re-certification MUST be received by January 31st.
